Patient Portal

Access your health records and upload your required health forms via the Patient Portal.  Caution: If you share your login information with friends/family, they will be able to access your records: https://aic.medicatconnect.com/ 

Please click on Health Requirements for Students to see a list of required health items needed prior to the start of classes.  Click on Physical Exam and Immunization Form to print out the forms that need to be completed by you and your medical provider.

Counseling Services: AIC offers free, confidential counseling services to all registered undergraduate and graduate students. Schedule an appointment through the Patient Portal or email Dexter.Counseling@aic.edu. Click here for more information. 

Visit Future Health for additional Wellness Information and Support: https://www.myfuturehealth.com/. Register for free using your AIC email address.

Mandatory Health Insurance Information: Visit University Health Plans for completion of the enrollment or waiver form. All enrollments or waivers must be completed online. This process must be completed annually.
