Forms	Forms: Complete Online Þ Financial Responsibility Agreement Form (Form #1: once every year) Þ Other forms are optional. You may submit them to make updates.
Health Insurance	Student Health Insurance: Waive or Enroll Þ All students are charged for the student health insurance. Returning students are charged in the Fall term. If you did not attend for the Fall term, you will be charged the Spring Only rate for teh Spring 2025.
